Insurance
Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.
We recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Does my health insurance plan include out-of-network coverage?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
Life Strategy Consultants, LLC is credentialed with a variety of Insurance Companies and Employee Assistance Programs. If you do not see your insurance provider listed below, you may be able to use out-of-network benefits. Additionally, you may contact the office to determine a self-pay rate if you do not currently have mental health benefits through your insurance provider, or if you simply choose to not involve an insurance provider in your mental health care.

Payment
We accept cash, check, all major credit cards, and Health Savings Account (HSA) debit cards as forms of payment. There is a $35.00 charge for checks that are not honored by your financial institution.
Cancellation Policy
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged a $75.00 fee for the missed appointment.
